Look on the trim tag for the numbers 66 that is the option code for a Formula. Here is a shot of my Yellow cars trim tag. The only other way to tell is by finding a build sheet in the car, or spending the $$$ about $60.00 to have Pontiac Historical Services run the paper work for you.
Other ways to tell if it is a Formula, there are a few Must have options, Power Steering, and Tilt Column among them. Other features are as you mentioned a Formula will have a Trans Am Style Formula Wheel (not 100% as my yellow car the original owner paid an extra $18.00 to not have on, it came with a 3 spoke delux steering wheel.) Rally Gauges, RTS Sunpension with front and rear sway bars, either Ralley or Snow Flake wheels (if Rally wheels they will have the bright trim rings around the 4 slots like the Olds and Buick Rally wheels.) And a wagon will have the rear air deflector.
Like I have said previously all I have are some old copies of pictures of a Black Formula Wagon from the original owner. Not very detailed.
